REMEO®- supporting patients and relatives after discharge from hospital

For most ventilated patients in a stable condition, homecare has become a realistic alternative to inpatient care. However, the option of discharge from hospital or from a REMEO Center is often a difficult decision because of the relatives’ concerns about returning home. Some carers may not know how to manage the patient’s care and, more importantly, how to ensure their safety.

 

Training for the challenge of homecare 

An important part of the REMEO programme is providing support for the family. This means preparing them for the challenges of homecare and teaching them how to carry out the necessary routine tasks, as well as what to do in an emergency.

Continuous support for homecare 

At home, the support offered by REMEO staff depends on the family’s requirements:

  • Multi-hour care: A REMEO nurse remains at the patient’s home for the requested period – for 24 hours, 7 days a week, if needed.
  • Occasional homecare: Regular visits by a REMEO nurse who reviews the patient’s condition, performs blood gas analyses and checks the ventilator parameters in consultation with the responsible physician.
