Where patients aim to ultimately return home, “REMEO®-I return home” centers bridge the gap between Intensive Care Unit (ICU) and the home. In this transition period we adapt the patient and the family caregivers to the new situation of staying outside of the “controlled ICU environment”. Simultaneously, our case managers build up the needed human resources and equipment to allow the patient to return home.
The REMEO® centre further can be used for respite care where a home-based patient’s carer becomes unavailable (e.g. due to a sickness or need for a holiday). Should the condition of a home-based patient slightly exacerbate, an admission to a hospital can be avoided by referring the patient back into the REMEO® Centre.
