There is clear evidence that suggests patients recover in their own home environment quicker. Being cared for at home also reduces the risks associated with hospitalisation. As a result, there is a clear trend towards home-based care of patient's requiring mechanical ventilation.
REMEO® provides a comprehensive treatment plan that focuses on a patient's quality of life while also promoting rehabilitation in the warmth of the patient’s own home, where they are surrounded by family and friends.
REMEO® – Welcome Home
Returning home after a long stay in hospital can be an emotional time for a patient and his or her entire family. Surrounded by loved ones, however, patients can gradually enjoy being back in a familiar environment with the knowledge that the highest standards of care and safety are being adhered to.
REMEO® welcome home services ease this transition by offering patients and families everything they need – from a single source. Welcome home services begin while the patient is still in hospital in order to ensure continuity of care at home.
REMEO® staff provide care in the patient's own home. Depending on the patient's individual needs, this can be in the form of 24-hour care, 7 days a week or multi-hour care.
Trusted partner for families and caregivers
At REMEO®, our staff encourage patients and families to actively engage in the care process. A crucial step in returning home involves developing the skills of carers and supporting them through ongoing training. This allows carers - depending on the complexity of a patient’s needs, to assist the patient in their daily activities.
Before a patient is discharged from a clinical facility, REMEO® gives the family and caregivers comprehensive and individual training to build the skills they need. This covers daily care routines as well as emergency responses.
